A medical services contract The True Health Plan is an annual medical services contract provided through an employer or association and True Health Medicine, PC. The plan provides affordable access to primary health care through the doctors of True Health Medicine, PC and supporting services such as diagnostic laboratory testing, and the peace of mind having that access creates. Terms and conditions There are no exclusions for pre-existing conditions. Individuals with chronic health conditions are invited to utilize naturopathic primary care, acupuncture and physical medicine services to enhance their health prospects and quality of life. For employers utilizing monthly payroll deductions, subscriptions are $40 per month for an individual and $80 per month for a family. Subscription fees paid monthly through a cafeteria benefits plan, also known as a section 125 plan, may qualify as a non-taxable payroll deduction. Consult your tax advisor regarding tax deductibility and specific requirements. For the self-employed, members of associations, and employers without a payroll plan, subscriptions are paid quarterly at $120 for an individual, and $240 for a family. Your tax advisor may know a way to structure your organization’s quarterly subscriptions so they can be paid with pre-tax income. Benefits There is no annual deductible with the True Health Plan. For plan subscribers all physician visits are $50 per visit, for the first visit, for every visit, for unlimited visits. The $50 fee applies regardless of the complexity of a visit or the amount of time a visit requires. Most laboratory testing paid through True Health Medicine is at a 30-80% discount from what patients pay the laboratory directly. Referal to fairly priced diagnostic imaging and advanced services as required and available. For those with an HSA (health savings account) MSA (medical savings account) or FSA (flexible savings account), expenses associated with doctor visits, diagnosis and treatment plans qualify for payment with pre-tax funds from those accounts. Consult with your tax advisor regarding other ways your routine health expenses might be paid with pre-tax dollars. |
